Long Term Care

Medical Services

A team of highly qualified Kaiser Permanente physicians and nurse practitioners manage the medical care needs of Kaiser Permanente members residing in skilled nursing facilities. They personally visit the members, and provide 24-hour on-call coverage. Continuing Care assistants provide in-office support and handle messages and phone calls. Benefit Management Services

Our Benefit Management Services are provided by Extended Care Coordinators (ECCs) who are nurses and physical therapists. They work together with the physicians, nurse practitioners, nursing home staff, Medicare intermediaries, families and members to monitor and manage members’ benefits while in the nursing home. These ECCs are available Monday through Friday at (619) 641-4456. The Kaiser Permanente team uses an innovative, outcomes-based program to assist in evaluating members’ progress in rehabilitation. The program helps the team project the length of stay needed in the nursing home and identifies appropriate levels of care following discharge.

Placement Services

Our Placement Coordinators are nurses who work with discharge planners, skilled nursing facility (SNF) admission coordinators, case managers, physicians, Extended Care Coordinators, families and members to place members in nursing homes. They assist with communication and coordination among the parties involved to make the transition as smooth and efficient as possible.
